Mill Neck Services
The Community Habilitation Program supports services to people with developmental disabilities who are Deaf, Hard of Hearing as well as hearing who are living on their own or with loved ones, that require assistance while learning independent living skills. Using our person-centered approach, we help each individual achieve and live a life that is full of possibilities and meaningful interaction, consistent with their abilities and needs.
Working together with the individual with disabilities, their families or others in their circle of support, we identify those areas where the individual may choose to enhance their levels of independence, such as: personal hygiene, money management, safe use of transportation, health and wellness, or social skills.
Once these areas are identified, a person-centered plan is created. Every six months, we revisit the plan to gauge how far we have come and where we need to go.
Do you have self-direction? No problem! We will work with you in conjunction with self-direction to most efficiently utilize your budget! Rely on our expertise to find you trained professionals with complete background checks, coordinate your schedule and trust in our quality standards.
